My current survey stack costs $0 to start and scales to roughly 10,000 responses per month before I pay anything. Tally is the form builder. Free tier covers unlimited forms and unlimited responses. Make.com routes the answers to Airtable. Free tier on Make is 1,000 operations per month, which fits a survey that gets 200 responses a week without trouble. Airtable stores everything. Free tier handles 1,200 records per base.

The rule I follow on every survey. Three questions maximum. Anything longer drops completion rate below 40% in my own data. Three questions, one open text, two yes/no or rating. People answer that on a phone while waiting for a coffee. Anything that asks for 12 minutes of attention gets abandoned.
